Do you receive funding for group activities?

At Prahran Place neighbourhood Centre we strive to create a vibrant, fun and caring community environment for everyone and provide fun and beneficial group activities for funded customers.    Are you:

  • aged 65 years and over or Aboriginal and Torres Strait Islander and 50 years?
  • aged under 65 or Aboriginal and aged under 50, with complex needs and not eligible for NDIS funding?

 

Prahran Place Neighbourhood Centre offers a range of activities suitable for funded Customers. As a funded Customer, you can elect to attend Prahran Place or be referred to Prahran Place to attend classes. Partial or full payment for classes will come from the funding allocation you receive from the Australian government’s CHSP or HACC schemes.

Commonwealth Home Support Scheme (CHSP)

If you are aged 65 years and over or 50 years or over for Aboriginal and Torres Strait Islander people, have registered with My Aged Care and been allocated Commonwealth Home Support Scheme (CHSP) funding for Social support – group activities to attend free and discounted classes at Prahran Place. 

Home and Community Care Program (HACC PYP)

If you are aged under 65 or Aboriginal and aged under 50, with complex needs and not eligible for NDIS funding, you may be eligible for Home and Community Care Program for Younger People (HACC PYP) funding to attend free and discounted classes at Prahran Place.

Discounts on classes

If you receive CHSP and HACC funding, you can attend health and wellbeing classes at a reduced cost. Popular classes include Gentle exercise, Zumba and Yoga and Meditation. For more information check out our website: https://prahranplace.org.au/health-wellbeing-and-exercise/

What is required to use this funding at Prahran Place?

As a social activity provider for a diverse customer base, we take meeting our duty of care and compliance requirements seriously. We will support you to access our services and understand the information we request and keep. We aim to make the documentation process easy as possible to maximize your time doing fun activities!

Services Australia customer service officers and Aged Care Specialist Officers can provide information about My Aged Care services

Elder Rights Advocacy can support you through the My Aged Care process, with accessing services and addressing issues you may experience with them

Prahran Place can submit a referral for you to sign up to My Aged Care

How else can Prahran Place help?

  • Offer guidance information on how to sign up
  • Sit with you while you call an assessor with your specific consent
  • Offer you use of PCs, phone and ipads to access information
  • Offer information posters, brochures and talks from service providers
  • Offer you classes and events at funded category prices and contribute to your goals through a Goal Planning process and recording regular observations on your progress, needs and preferences
  • Refer you to service providers that provide specific care and support

INTRODUCING PRAHRAN PLACE PLAYGROUPS!

Playgroups are regular and relaxed meetings for groups of young children who have not yet started primary school.

Tuesdays or Thursdays, 9:30-11:30
