Under the Home and Community Care Program for Younger People (HACC PYP) funding is provided by the Victorian Department of Health and Human Services for eligible younger people (people aged under 65 and Aboriginal people aged under 50 with complex needs and not eligible for NDIS funding) to receive services and support for their assessed needs.
In the city of Stonnington, assessments can be carried out by mecwacare. You may be eligible to receive funding for group programs at PLCL for physical activity, cognitive stimulation, good nutrition, emotional wellbeing and social inclusion
HACC PYP funding for participation in “planned activity groups” can be accessed by eligible people to participate in social group programs at PCLC.
